We are in the process of making our forms available to us online. For some time, this is going to be a work in progress. If you have any suggestions as to forms you want on this page or any other feedback, please feel free to send me an e-mail. With all of the usable documents, you should be able to open them in your internet browser where they can be edited and printed. you can also save the document to your local hard drive and edit them in your word processor. Conditional Plea Forms
The Motion to Enter a Conditional Plea is the document you can enter after losing an Omnibus Hearing to plead guilty but preserve your issue for appeal. Once the Prosecutor and the Judge have signed off on the above motion, you and your client can file the petition to enter a conditional plea. It is similar to our present petition to enter a plea of guilty forms.
Out Of State Witness and Document Subpeonas
Clara Rigmaiden has prepared a guide to navigate the forms and procedures necessary to subpeona a witness or documents from out of state. There is a guide for witness subpeonas and a guide for documents subpeonas. They have been modified with hyperlinks so that as you peruse them, you may link directly to the forms being discussed.

As this is the first “beta version” of the forms page, this is what we have up and running so far. More forms will be placed on here as time allows. Remember, if you have any forms you want posted, e-mail me. Also remember, it helps TREMENDOUSLY if you have that form available.
Also, there are some more forms available through the OCDLA at their website. You will need your OCDLA username and password. If you don’t have one or forgot it, you can call them. Once you link to the OCDLA website, there is an option to the left for their “legal document library.”
